Introduction: Traditionally in psychiatry homosexuality is believed to be pathological. In spite of its exclusion as a mental disorder psychiatrists frequently exhibit covered or explicit homophobic attitudes. The term “homophobia” is not included in the most widely used medical browsers and it is not found in psychiatry textbooks, where homosexuality is still considered a mental disorder. Methods: Narrative review of the literature. Conclusions: Homophobia is a poorly studied mental disorder in psychiatry. Some of the possible actions towards reversing this tendency would be to support research endeavors, the inclusion of this topic in medical under and postgraduate curricula, the development of guidelines and safe zone projects for sexual minorities. An epistemological and political change in Colombian psychiatry is needed in order to establish a statement both as a group and individually about different forms of antihomosexual prejudice, homophobia, and the disclosure of therapists’ attitudes towards sexuality.
