The recurrence of tuberculosis (TB) becomes a diagnostic challenge because in many instances the immune system may be altered such as immunodeficiencies. There is another scenario in which a different strain of Mycobacterium tuberculosis can reach the outside and cause the disease again in independent patient of their immune status, the most frequent case in immunocompetent patients. The ratio of reinfection with immunosuppressive condition of the patient takes into account the behavior of prevalence and incidence of the disease, in the case of HIV to high prevalence is the incidence of tuberculosis, for which there is the coexistence of infection. The current challenge alert to have a greater suspicion against certain cases that deserve studied in depth by other techniques, looking for a reactivation or reinfection, taking into account the clinical, social and economic impact that it generates.
