Transverse myelitis is the spinal cord injury that causes complete deficit (sensory and motor) from the level in which it is present, classically associated with the loss of sphincter control. There is a variant known as longitudinally extensive, which in magnetic resonance imaging (MRI) affects three or more vertebral segments. We present the case of a patient with a medullary syndrome secondary to neurosyphilis with folate deficiency and extensive spinal cord injuries.
