A thunderclap headache (TCH) is defined as an excruciating headache which occurs abruptly like "thunder," reaches its maximum point in less than a minute, and should be considered a medical emergency.
Below, we present the clinical case of a 49-year-old Colombian patient who developed a thunderclap headache as the initial sign of pituitary apoplexy. He progressed satisfactorily, with no evidence of sequelae, highlighting the early diagnosis of a potentially fatal entity. (Acta Med Colomb 2022; 47. DOI:https://doi.org/10.36104/amc.2022.2336).
