Hemophagocytic syndrome is an entity caused by hypercytokinemia and impaired immune system, may be hereditary or acquired and generates high mortality despite treatment. Among the etiologies, infections occupy an important place, one of them disseminated histoplasmosis, an opportunistic infection of difficult diagnosis which affects immunosuppressed patients. We present the case of an adult patient, with renal transplant who developed hemophagocytic syndrome secondary to disseminated histoplasmosis and responded adequately to the antifungal treatment.
