Non-Hodgkin lymphomas are a group of neoplasms that affect the lymph nodes and / or extranodal lymphoid tissue. Its frequency has been increasing in recent years. According to the WHO classification of non-Hodgkin lymphoma, splenic marginal zone lymphomas (SMZL) are neoplasms of mature B cells that constitute less than 2% of all these types of lymphomas. The case of a 58 year old patient who consulted by presenting prolonged fever associated with weight loss, pancytopenia and painless massive splenomegaly, is presented. After conducting multiple studies, which were negative, it was decided to perform diagnostic and therapeutic splenectomy. Histology result is obtained is consistent with splenic marginal zone lymphoma. Subsequently, the patient presents successful improvement of all symptoms and is discharged from the hospital.
