Objective: To evaluate the analgesic effect of tramadol compared with meperidine in the postoperative period of patients programmed for knee surgery. Material and methods: We were carried out a controlled clinical trial, double blind, at the hospital “Guillermo Luis Fernández Hernández-Baquero”, Holguín, Cuba, during the months of February to August of 2007; 84 patients were selected, which were randomly divided in two groups of 42 patients each. We applied lumbar epidural anesthesia and a catheter was placed. Two per cent mepivacaine was employed in a 5 mg/kg dose. Ten minutes afterwards we administered through the catheter tramadol (100 mg) to group I patients and meperidine (100 mg) to group II, dissolved in 10 ml of 0.9% physiologic saline solution. Heart frequency and transoperative and postoperative mean blood pressure were determined. Pain evaluation was carried out by means of a analogue visual scale. We carried out the measurements in three periods occasions: recovery, and at 2 and 4 hours after the surgical procedure. We registered the most frequent secondary effects. Results: The 4 hour evaluation after the surgical procedure reported analogue visual scale values below 4 for both groups, without any statistically significant differences (p=0.31). The most frequent adverse effect was nausea in Group I (16.6%) and in group II (28.5%). Conclusions: Epidural analgesia with tramadol and meperidine prolongs analgesia in patients with surgery for knee pathologies.
