Within the health quality processes, in order to reduce the risks in the care of the user population, the patient safety policies have captured the world's attention. These policies emphasize the importance of preventing, controlling and intervening unintended injury or complication called adverse events, that may occur during care and must be reported. However, the active identification of adverse events in institutions such as the one where we undertook the present study is much higher than those defined by the self report, that is, which has shown a situation of underregistration of adverse events and in terms of quality this should be improved, but not before discovering the causes that generate it. In this sense, the present study was performed to describe the causes of non reporting adverse event through a mixed research method. That allowed to Identify that there are causes "invisible", such the fear presented in health staff, which is subjective and has the ability to influence and determine the adverse event reporting. This research is consolidated with the perspective of intervening a situation even to prevent them is an essential requirement to determine the causes, wherefore, this study makes an important input for future interventions and implementation of the so called reporting culture and patient safety.
