Male cancer of the breast exhibits a very low incidence and although its natural history and histologic characteristics are similar to female breast cancer, its epidemiology and hormonal climate are different. We have studied and treated a 72 year- old man who presented at Hospital Susana López de Valencia, in Popayán, Colombia, with a palpable mass in the left mammary region that had been present over the previous six months. Following histologic diagnosis of invasive ductal carcinoma, a modified radical mastectomy with axillary dissection was performed. Sections of the surgical specimen were stained with hematoxylin-eosin and immunohistochemistry for hormonal receptors, which were positive for estrogen and progesterone. We have considered important to review the literature pertaining the less usual and most controverted aspects of male mammary cancer, such as epidemiology, genetics, tumoral markes and hormonal climate, which are all of fundamental importance for an optimal management, and which deserve future prospective studies.
