Paracoccidioidomycosis is a systemic mycosis caused by the dimorphic fungus Paracoccidioides brasiliensis. The disease occurs in Latin America. It is the main systemic mycosis in Brazil, with the highest incidence in the southern, southeastern and central regions. The disease is acquired by the inhalation of the fungus, which spreads to other organs through linfohematogenic dissemination; it has two forms of presentation acute or subacute which occurs in children and adolescents. The chronic form occurs in adults with focal or multifocal manifestations. This paper present a male patient aged 51 who debuted with paralysis of VIth cranial nerve, documenting multiple subcortical lesions, associated to with pulmonary, lymph node and adrenal glands involment. Histopathology showed evidence of paracoccidioidomycosis.
