Introduction: The postpartum period is a time of contrasts and adaptations which are required of family support and the care of nursing professionals. Materials and Methods: This article is the result of implementing a strategy for nursing homecare for women undergoing postpartum week based on the five processes of care theory Swanson care, leaning on the logical frame work as a strategy for planning, execution and evaluation. Through a nursing situation shows the different needs of mother and child at this stage and the need for support from nursing. Results: As a result of this intervention were identified a cultural diverse practices which endanger he mother during the postpartum period, also the ignorance of many care practices performed on both the mother and the newborn. Discussion and Conclusions: The care of nursing is essential in every phase of postpartum week, as many of the complications in the mother and the newborns occur at this stage. At present the health legislation provides the opportunity to get much closer to the mothers of new born through the establishment of primary care model which aims to bring health personnel to the contexts and realities experienced by the community.
