Neuroinfection by Listeria monocytogenes, which includes meningitis and rhombencephalitis, has traditionally been described in people at the extremes of life (under one year and over 50 years), pregnant women and immunosuppressed patients with dysfunction of cellular immunity. The compromise of young patients without these predisposing conditions is rare. The elevation of the adenosine deaminase (ADA) in cerebrospinal fluid (CSF) has been described as a test with high sensitivity and specificity for the diagnosis of tuberculous meningitis, but not for the meningitis caused by Listeria monocytogenes.

The case of a young female patient with no known immunosuppression who presents a clinical picture of subacute meningitis, CSF with mononuclear pleocytosis dominance, VI cranial nerve paralysis and elevated ADA secondary to L. monocytogenes compromise is presented.
