Abstract In this paper a reflection on the concepts and importance of nursing notes is made; this shows their interest to foster communication between interdisciplinary team providing a clear, accurate and detailed care act, to articulate the process of nursing care. a reflection of reality in terms of the delegation of functions of the profession is exposed, showing the lack of leadership and loss of autonomy by the professional; nursing annotations are taken as the written support of care provided in professional practice; the authors make an invitation to resume the theoretical and methodological foundations of nursing notes and re-signification in order to give visibility and strengthen leadership and management act of caring.
