Obtaining high anesthetic efficacy in mandibular molars with pulpitis has been a complex process, given the combination of multiple factors, including the pH of the medium and the technique to be used. The aim of the present review was to compare the available evidence on the anesthetic efficacy of the inferior alveolar nerve block in molars. Therefore, a systematic search of the literature was carried out using the information seekers: PubMed, Science-Direct, Scopus, Ebscohost /Dentistry & oral sciences source and ClinicalKey. The studies were selected through analysis and the checklist of the PRISMA format, those systematic reviews whose objective was to evaluate the effectiveness of the inferior alveolar nerve block in molars with pulpitis were chosen, seven studies met the inclusion criteria. Data extraction was performed by critical reading of the previously selected systematic reviews. The results of this umbrella systematic review show positive effects in terms of anesthetic efficacy when performing inferior alveolar nerve block in cases of irreversible pulpitis, the effectiveness between studies varies depending on the anesthetics used. Concluding that many factors can affect the concordance of the results, although the use of IANB in cases of pulpitis in molars shows good results, it still does not reach an anesthetic success rate higher than 80%, regardless of the use of articaine which was the anesthetic with the best results in most reviews.
