It is estimated that among 500,000 to 1 million deaths occurred annually worldwide due to complications associated to hepatitis B virus infection (HBV). The HBV has the higher mutation rate among DNA viruses that infect humans. Although some HBV genotypes have been associated with clinical complications during chronic infection, recently published data suggest a stronger role of HBV variants. Precore/core region and its promoter element are one of the HBV genomic segments with a higher mutation rate. The double mutation (A1762T/G1764A) in basal core promoter sequence results in dramatic viral replication changes and poor response to antiviral treatment. This review summarizes the present knowledge about the genotype and precore/core variants influence in clinical outcome, immune response evasion and antiviral therapy.
