This study, through an integrative review of the scientific literature, analyzes how the mental health care practices include the dimension of citizenship, from the strengthening and/or construction of its workout. It discusses the current scenario of new mental health care practices, developed from the psycho-social paradigm, based on the propositions of the Brazilian psychiatric reform, which features citizenship as a centerpiece in the formulation of policies and and the creation of the mental health care network. To this end, we analyzed Brazilian articles published in the Lilacs data base/BVS and Scielo, during the period from 2002 to 2011, which presented practices of mental health care within the health system, in terms of how they are developed, their advances and challenges. The results indicate that, currently, a wide range of new practices are developed in different services and by multiprofessional teams and that these bring important innovations to deal with the madness. However, the asylum model is still present, expressed in new clothes and developed for new services, as well as discontinuity of practices and lack of funding for the development of innovative activities.
