The large increase in the demands of sports and physical activity in population has increased the number of primary anterior cruciate ligament (ACL) surgery. The study identified the potential causes of instability recurrence in a group of soccer players undergoing revision surgery. They were studied retrospectively 17 cases of ACL revision surgeries, from a total of 629 primary stabilization operations of the knee, in the time between July 2000 and May 2004 in the Spanish Footballers Mutual Delegation of Catalonia. We evaluated demographic and radiographic studies, and monitoring protocols of clinical and evolutionary assessment. The main cause of failure was the trauma, found in 94.1% of cases, followed by technical surgical errors, still prevailing the poor location of the femoral tunnel (58.8%) and tibial tunnel (43.9%). Aggressive rehabilitation and new traumas must be avoided in patients operated of ACL, exposed to high energy interactions. Moreover, it is recommended to include a radiological assessment of the situation of bone tunnels, in both the femur and the tibia in the lateral radiographic, within assessment clinic protocols of the anterior cruciate ligament plasty.
