This research aims to analyze the relationship between public spending and services to the population with basic sanitation in cities. To conduct the study were collected data on water, sewage and waste, which were faced with public spending of 185 cities in Brazil and Perú in the year 2014. The data were processed by means of data envelopment analysis (DEA), and used as input data on spending on sanitation and as outputs the information on drinking water supply, waste collection and treatment of wastewater. The results showed lack of care to the population of the three items analyzed, which may lead to social and environmental problems. Moreover, it was not possible to relate spending on sanitation and services to the population, as cities that spend more (per capita expenditure) reveal no higher level of service to the population. Lack of sanitation can be related to weaknesses in public policy and can have negative externalities such as disorder of urban environments, public health issues, contamination of the Atlantic Ocean (Brazil) and Pacific (Peru), contamination of groundwater and rivers both affecting the environment as the population.
